Summary


Applications are invited for the role of Trust Grade Doctor in General and Geriatric Medicine under a high level of Consultant supervision.

The post is graded at the equivalent level to Core Trainees (CT1-3) and you will be placed at the appropriate level based on your experience. Although graded at Core Trainee level, the post is not recognised for training by the Postgraduate Dean, however, many of our Trust Grade Doctors successfully go on to join formal Training Schemes in the future.

This post is ideal for a Doctor wishing to develop clinical skills and consolidate their knowledge in this area of work.

You will benefit from gaining valuable experience and will also have access to several training/educational opportunities including dedicated teaching sessions. The post will suit an individual considering a career in the NHS and offers close support and supervision from a Lead Consultant and our Director of Medical Education.

The successful applicant will work in a friendly multidisciplinary team together with another Junior Doctor with direct supervision from the Lead Consultant Dr Gloria Yu, in the 36 bedded Meadow View Intermediate Care Unit based in Queen Mary’s Hospital, Sidcup, Kent.

This service specializes in the management of Frailty related conditions and Rehabilitation.

The local population contains a significant level of frailty with high rates of dementia and associated delirium, so there will be many opportunities to experience a rich mixture of pathologies and complex psycho-social issues.

The postholder will also rotate to work in the Bexley Rapid Response Team as well as the Bexley Frailty Virtual Wards which will provide excellent experience in managing acute and chronic frailty related conditions in the community while working with primary care.

There are no on call responsibilities associated with this post.

The Job Description for this post is based on full-time post but can be adjusted accordingly for part-time Doctors.

There is no on call responsibility attached to this post.

The duties of the postholder will include:
• Attending all Consultant rounds and daily handover meetings
• To communicate and work collaboratively with colleagues to maintain / improve patient care.
• To explain the diagnosis and management plan to the patient and their families
• Comprehensive geriatric assessment (CGA) of all new patients in Meadowview
• Arranging the day-to-day care of 36 inpatients together with the other ward doctor and managing acute and chronic pathologies under Consultant supervision
• To be responsible for maintaining a high standard of medical case-notes, ordering, and interpreting investigations (informing the Consultant of abnormal results) and making referrals to other specialties.
• To ensure that discharge summaries are completed promptly and accurately.
• The post rotates to the Bexley Rapid Response Team and Frailty Virtual Ward where the doctor is expected to visit patients in their home or Care Home to carry out a CGA and make a diagnosis and management plan under Consultant supervision. Patient to be monitored until discharged with a formal summary to the GP.
• There are no on call responsibilities.
• To undertake CPD activities, CBD and Mandatory training provided by the Trust.
• To participate in audits, QI projects and teaching
• To have a yearly appraisal conducted by the Lead Consultant
• Requests for annual and study leave will need to be submitted and approved by the Lead Consultant at least six weeks beforehand.
• The description of the post cannot be exhausted and the postholder may be requested to undertake, exceptionally, to perform additional duties in occasional emergency and unforeseen circumstances following discussions with the Lead Consultant
• The postholder will comply with the Trust’s agreed policies and procedures to practice in line with the Royal College of Physician’s Best Clinical Practice Guidelines and the General Medical Council’s Good Medical Practice Guidelines.



Workload and Responsibilities

The role and main duties include to undertake comprehensive medical and mental health assessment (CGA) of all newly admitted intermediate care patients (approx. 400 patients per year). The new 36 bedded Unit is anticipated to admit around 600 patients a year.

CGA required to detect pathologies undetected in secondary care which may require immediate medical treatment or limit rehabilitation potential. Without a medical CGA, more patients would deteriorate on MVU and require transfer back to secondary care.

As part of the team, you will be responsible for the day-to-day medical care of the 36 beds on the amalgamated Bexley and Greenwich Units. These are frail patients prone to acute medical deterioration such as infections, falls, and unstable diabetes. The presence of a medical doctor allows early diagnosis and treatment on MVU especially as we now have the capability to give IV antibiotics. Recently due to the increased pressures on the acute hospitals, patients being transferred are increasingly more complex with unresolved medical and acute mental health problems requiring more medical interventions from the junior doctors.

You will be an integral part of the MVU MDT providing medical support to the ward staff and medical care and information to the patients and their relatives as they commonly do not know or fully understand all their medical diagnoses or prognosis.



Induction

You will receive an induction to introduce you to the Trust generally and you will also receive local induction into the individual workplace and be informed of the requirements of your post.

A Medical Staffing Officer is also assigned to your area of work for Bexley and will be able to support you during your time with us.



Clinical Supervision

The post holder will be given clinical supervision by the nominated Local Consultant.



Contracted Hours

You will be required to work Monday to Friday, 40 hours per week.

The post will be covered by the terms and conditions of service for Hospital Medical and Dental staff (England and Wales) as amended from time to time. Contracts have been agreed with the BMA local negotiators, but individuals may wish to discuss this further before acceptance of an offer of appointment.
