Summary
Reporting the physical health checks back to the GP and if any have any concerns with physical health IE Blood pressure concerns, raise accordingly within the team but also the GP to continue to monitor if any blood pressure concerns needing further investigation/treatment through the GP in primary care. You may also be required to support other colleagues in the team on two person visits, you will not be holding a caseload but would be expected to help and support other colleague in the team on two person visits if required.
