Summary

The following are the primary responsibilities of the Long-Term Condition Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: A. Work collaboratively with the practice team to proactively identify and manage patients, which includes patients with long-term health conditions, QOF indicators and IIF indicators and patients on other registers that require input. B. First point of contact to assist patients manage their needs through making and managing appointments, active information co-ordination, answering queries, and ensuring that people have good quality written or verbal information to help them make choices about their care. C. Ensure consistent, high quality and comprehensive monitoring and recording of information onto SystmOne as well as other templates as required to ensure data capture meets the needs for the QOF, DES, PCN, IIF, etc. D. Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing. E. Provide coordination and navigation for people and their carers across health and care services, working closely with other primary care professionals; helping to ensure patients receive a joined-up service and the most appropriate support. F. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing. G. Support and manage the Carers records and reviews at the practice. H. Signpost and support patients to other services such as the social prescribing service to help patients access services to support them. I. Support the practice to keep care records up to date by identifying and updating missing or out-of-date information. J. Support the Operations Manager in the writing of related policies, protocols, and user guides. K. Conduct system searches as requested. L. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately. M. Communicate with line manager any queries relating to job role such as coding, summarising, recalls, etc. to obtain clarification. N. Support all clinical staff with general administrative tasks as requested. O. Log any IT faults with helpdesk, ensuring that details and updates are shared with those involved. P. Undertake other relevant duties when requested by line manager. Q. Attend practice, team and other meetings when required. R. Work effectively as part of a team. S. Support the Management team with colleague training when requested. T. Photocopy documentation as required. U. Share information, skills and experience with team members. V. Maintain a clean, tidy, effective working area at all times. In addition to the primary responsibilities, the Long-Term Condition Coordinator will be required to: Provide cover for the Coder/Summariser when required (annual leave, sickness, etc.) Provide cover for the Reception Team when required (annual leave, sickness, etc.)
