Summary

The Team The Pharmacist Network are a clinical led organisation here to support GPs with the delivery of pharmacist and pharmacy tech services. We employ pharmacists andplace them into PCNs and in partnership we support the training, development and workflow management of the pharmacists. We currently have an exciting full-time opportunity available for highly motivated and focused Clinical Pharmacists to support with the smooth running of day-to-day activities in our general practice within the Knowsley PCN. The Role As a Clinical Pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists. You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare. You will support practice staff with prescription and medication queries. Main duties of the job As a clinical pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's. Resolve medication-related issues Helping to utilise skill mix Improve patient outcomes Ensure better access to healthcare Support practice staff with prescription and medication queries Medicines reconciliation Provide expertise in clinical medicines advice Providing clinical leadership on medicines optimisation and quality improvement Previous primary care experience and being proficient with EMIS or System 1 is essential, although as this role is onsite, training can be provided / supported. Desirable to already be on CPPE Primary Care pathway or will be willing to partake if needed, if you have extensive experience then we can look into getting an exemption for you.
