Summary
Working within the Integrated Discharge Service (IDS), as an integral part of the administrative team providing support to the IDS team, facilitating the discharge of patients with complex needs from the Trust, processing referrals to community to provide safe Discharge home and general admin tasks
The post holder will be a part of the Admin team who are an essential link between the Hospital and Community providers providing a 7 day service.
The post holder will be required to have existing experience in administrative, IT and clerical processes and procedures, and experience of working within a team model, supporting resilience and service delivery.
• Processing referrals to community for discharge
• Answering the telephone and dealing with enquiries in a polite and professional manner
• Maintaining a variety of spreadsheets and IT systems ensuring data is accurately recorded
• Liaising with wards and external partners to inform them of discharges and to problem solve when required
• Escalate issues to Team Leads
• Attend meetings and record actions
• To work as part of a team sharing responsibility for the smooth running of the service.
