Summary

In order to meet the needs of patients in our prisons, Practice Plus Group haves developed a n exciting new integrated approach to the provision of care. Teams will work together at each stage of the patient journey, supporting them as they arrive in HMP Pentonvillethe prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team). Prisons have a dedicated team of staff whose job it is to keep the prison safe for everyone, called the Safer Custody Team. They have a specific responsibility for managing the risks of people harming themselves or other people, or of being harmed by others. The Safer Custody Practitioner will work as part of the Safer Custody Team to promote patient safety and safeguarding across the establishment. Assessment, Care in Custody and Teamwork (ACCT) is a framework to reduce risk whilst supporting the individual in a time of Crisis. This framework links with the Prison Service Orders and Prison Service Instructions in custodial settings. In Custody, a vulnerable patient is an adult who is unable to protect himself from abuse, neglect, or exploitation because of a mental or physical impairment or because of advanced age. The Safer Custody Nurse roleYou will be overarching across New Models of Care (NMOC) . Wworking with the multidisciplinary team to review and assess individual patient risk throughout the time in custody. Practice Plus Group will provide you with a rich and grounded induction that will enable you to build your confidence and knowledge of delivering healthcare within a custodial setting. This role will be key in liaising with other healthcare and prison colleagues in managing and reducing risk... Training will include Assessment, Care in Custody and Teamwork (ACCT) training and Case Manager training. Risk assessment, Suicide, and Self Harm (SASH) Training . Iin addition to any other relevant courses identified to support you and this role. You will be expected to attend all high risk ACCT first case reviews, to provide a healthcare perspective that will assist in the care map to provide support to the individual in crisis. Assessment, Care in Custody and Teamwork (ACCT) is a framework to reduce risk whilst supporting the individual in a time of Crisis. This framework links with the Prison Service Orders and Prison Service Instructions in custodial settings. In Custody, a vulnerable patient is an adult who is unable to protect himself from abuse, neglect, or exploitation because of a mental or physical impairment or because of advanced age. Complex individuals including those on a constant watch will require enhanced care planning to reduce and manage risk. These care plans whilst initiated by healthcare will form part of the overall strategy to support the individual. You will be a support to family liaison officers and link in with our regional Patient Safety Clinical Quality Lead to ensure robust measures are in place for deaths and incidents. You will be involved action planning following on from deaths in custody and serious incidents, and will lead in clinical case reviews. You will provide a strong link with hospitals when our patients are out for long periods of time with complex health issues. You will liaise direct with hospital staff, HMPPS and healthcare managers. You will attend all safety meetings such as the weekly Safety Intervention meeting, monthly Safety meeting and Use of Force You will contribute to our nursing strategy and vision to: Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them. You will be a support to family liaison officers and link in with our regional Patient Safety Clinical Quality Lead to ensure robust measures are in place for deaths and incidents. You will provide a strong link with hospitals when our patients are out for long periods of time with complex health issues. You will liaise direct with hospital staff, HMPPS and healthcare managers. Deliver non-judgemental care that makes a real difference, as defined throughout our bespoke Health in Justice Competency framework. You will be involved action planning following on from deaths in custody and serious incidents, and will lead in clinical case reviews. Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of. Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning. Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams and supporting each other to do a great job. You will attend all safety meetings such as the weekly Safety Intervention meeting, monthly Safety meeting and Use of Force.
