Summary

This role will play a fundamental part of Wakefield Place Health and Care Partnership Primary Care Team, supporting the delivery of our local Strategy across our 34 GP practices and 7 Primary Care Networks. The Primary Care Team is responsible for all elements of the delegation of General Practice commissioning for Wakefield Place. This includes Contract management / monitoring, Service specification development and Supporting Primary Care Networks. Additionally, the Primary Care Team plays a key role in the transformation of primary care services and the ICS (Integrated Care System), with the delivery of local place-based delivery plans working in collaboration with partners. You will bring to the role knowledge and experience of general practice and primary care networks with a commitment to ensuring delivery of high quality and consistent services meets with the needs of our patients. Main Duties and Responsibilities The post holder will lead the Primary Care Team to support the continued development of Primary Care Networks, where required, within a defined area, ensuring plans are consistent with the Primary Care Strategy and commissioning priorities. This includes working with clinical and managerial leads and others within the Wakefield Place and the West Yorkshire ICB ensuring appropriate collaborative working between designated localities and across all geographies, including Local Authority. The post holder will help to ensure that appropriate organisational developments take place across primary care and primary care networks within a defined area, including the development of skills, knowledge and capacity to achieve system maturity. Through the process of commissioning, co-commissioning and performance monitoring providing high quality care including the delivery of national plan targets, local targets and key national standards relevant to primary care. Primary Care Development To develop positive relationships with GPs and Practice Managers, clinical directors and business managers, local secondary and community service providers and the local authority in order to deliver primary care services. To lead on the delivery of a number of primary care projects and programmes of work, ensuring delivery against agreed objectives and ensuring that best practice is shared across the localities, ICB and the wider NHS organisation as appropriate. To provide support to the ICB Clinical Leads and primary care team to develop the Primary Care Strategy. To lead on and/or provide support for primary care projects on a matrix basis across the West Yorkshire ICS (where required). Responsible for project planning processes from inception through to delivery. This includes monitoring, reporting, managing risks and issues. To work with the Public and Patient Engagement Groups to ensure they are represented within the ICB, Primary Care Networks and General Practice. Participate in relevant internal and external working groups/projects, services and initiatives linking with the Business Intelligence team in order to provide information and analytical advice and expertise. Support commissioning processes specifically linked to primary care. Working closely with the clinical leads on specific pieces of work as part of the primary care strategy delivery. Primary Care Performance Work closely with practices in understanding data, systems and processes to increase knowledge, develop and spread best practice. Establish systems and processes to monitor quality in primary care. To interpret data in order to monitor practice performance, make meaningful clinical decisions and transform services/ pathways. To contribute to the development and delivery of strategies that deliver continuous improvement in primary care Primary Care Commissioning Working with the Senior Primary Care Development and Commissioning Managers and Contracts Management team, the post holder will support the delivery and monitoring of local contract development and performance and agreeing remedial action where required Provide oversight and monitoring of all aspects of Primary Care contracting e.g. Enhanced Service budgets Primary Care Commissioning Working with the team, the post holder will support the delivery and monitoring of local contract development and performance and agreeing remedial action where required. To carry out Contract Assurance visits. To respond promptly and efficiently to queries and issues raised by colleagues, member practices and both internal and external stakeholders, making decisions on appropriate actions as required. Project Management To work autonomously and with significant discretion to deliver a number of projects to support the work of the ICB, ensuring that project management arrangements are appropriate to the size and complexity of the project being undertaken. To produce business cases, service specifications and performance monitoring systems for the proposed service changes. To implement service change, evaluate the outcomes of service change and present findings and recommendations from these evaluations. To develop, propose and implement improvements to policies and processes that impact within and beyond own area. To interpret complex data and implement national policy, regulation and guidance relating to General Practice, translating this into ICB wide policy as appropriate. To handle complex facts or situations which require analysis, interpretation, comparison of a range of options and the ability to make recommendations.
