Summary


Lancaster and Morecambe Community Mental Health Team (CMHT) are looking to recruit a qualified Band 5 Mental Health Nurse to work predominately within our Physical Health Monitoring Team. The applicant would need to be registered with the registered body NMC. This is a permanent 37.5 hour contract. We would consider part time work applications.

The team serves the population of the Lancaster and Morecambe area. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.

This is an interesting and rewarding role in a busy service. Ideally, you will have current or previous experience in a community setting and be able to work effectively in a multi professional context. You should have a collaborative and enthusiastic approach as well as passion and commitment in developing and promoting recovery based approaches for adults with severe and enduring mental health needs.

The post requires that you are a good communicator, and are able to adopt a flexible, adaptable and proactive approach to your work.

The main job role is to perform duties as a Community Mental Health Nurse and to undertake assessments for people who are referred into the service for physical health monitoring when commenced on anti-psychotic medication including depot and Clozapine.

You will develop care plans and deliver evidence based interventions and treatments to patients alongside managing risks in the community for patients who are under the CMHT for shared care and depot and Clozapine monitoring. You will be working under the supervision of the Band 6 Community Mental Health Nurse within the Physical Health Monitoring Team and you will also have the opportunity to supervise junior staff.

This role involves managing unplanned patient work within the team, responding to escalation of risk/crisis situations and if required to step up care to a CMHT Band 6 Practitioner to assess.

You will benefit from encouragement to continue to develop your own specialist knowledge and competence. Your development will be guided using clinical supervision, management supervision, Team/Peer support, internal and external training opportunities and the Trust’s appraisal process. If you are a newly qualified Nurse then you will benefit from our Trust’s Preceptorship programme allowing you to learn about all of the aspects of the Team to ensure good development opportunities.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
