You will be responsible for the completion of Care Act Assessments to facilitate a smooth transition for service users when they leave hospital and for supporting discharges from intermediate care bed / inpatient rehab settings. Therefore, knowledge and experience in the completion of adult social care assessments is essential. As part of this role, you will have input in discharge planning with health colleagues, but also to ensure that the right pathways are achieved for the individual and a strengths-based approach is promoted as standard. The role entails working closely with individuals, families, carers and community services such as reablement, telecare, care homes and domiciliary home care providers to help individuals promote independence and safety when they leave hospital / health settings.
Key Responsibilities:
Participate in supervision and team meetings.
Participate in the department's duty arrangements, utilising relevant policies and criteria.
Undertake clear and detailed service user and carer assessments/reassessments and reviews in accordance with relevant statutory requirements, identifying risks, needs and options.
Manage the planning and review of cases, adapting and creating innovative packages to meet needs, promoting independence and early intervention as well as the personalisation agenda and self directed support to minimise risks, and commissioning services to improve quality of life for service users and their carers. Work with individuals, families, carers and communities to help them make informed decisions, enabling them to clarify their needs, construct value for money care packages and commission a range of appropriate services. Undertake preventative work and risk assessments utilising community resources and services such as reablement and telehealthcare. Input, check and maintain a variety of electronic and paper based systems including case notes and record systems. Prepare and present cases as appropriate to the Resource Allocation Panel in line with policies and procedures, as well as give evidence in court as required. Liaise and work with a variety of internal and external stakeholders, developing effective relationships to holistically meet the needs of service users and carers. Identify and act on opportunities to contribute to the development of practice champion in a specific area or taking initiatives to propose or try out new approaches.

Qualifications:
Relevant Level 3 Qualification in Health and Social Care.
Knowledge:
Knowledge and experience in producing quality assessments and experience of collaborating with various agencies.
Demonstrate knowledge of the needs of older people, adults with disabilities/mental health and vulnerable adults.
Demonstrate understanding of the assessment process and the contribution m/I
Demonstrate understanding of equal opportunities issues and anti-oppressive practice.
Demonstrate understanding of computer applications relating to assessment and delivery of service to users and carers.
Experience:
Some experience and understanding of the current Discharge to Assess process in relation to assessing / supporting people when they leave hospital.
Experience of working collaboratively with health colleagues, as the role does entail working in partnership with health in various different acute and intermediate care settings.
Some relevant experience of working with adults
Experience of working in a caring profession with older people and/or adults with disabilities/mental health.
Experience and/or knowledge of the skills required to complete assessments of need.
Skills & Abilities:
Demonstrate effective communication skills both verbally and in writing.
Demonstrate skills in forming effective working relationships with service users and their carers.
Demonstrate skills in work prioritisation, time management and working to deadlines.
Demonstrate ability to work as part of a team.
