Summary
Provide a clinical service to patients in a range of settings including community-based clinic settings and in-patient units, child development centres, schools and patients homes as appropriate. Be part of the clinical assessment and overall planning for the young person, which will involve attending weekly cases discussion and referral meetings whilst taking part in MDT (multidisciplinary meetings). Support the transition of patients and carers to adult services as appropriate. Promote optimal nutrition for patients on the caseload. Manage clinical risk and be individually responsible for a clinical caseload of patients with a range of complex conditions and diverse needs, requiring competent clinical practice for outpatients and patients at home, accessing clinical supervision on a planned basis. Negotiate and agree dietary or nutritional goals with individual patients and /or their carers using motivational interviewing, counselling and cognitive behaviour therapy tools as applicable. This may include identifying individual barriers to change and helping clients to manage them.
