Summary
Sunderland Care and Support is seeking to recruit a waking Night Care Co-ordinator (Level 1) for Farmborough Court Intermediate Care Centre. We are looking for someone who can deliver exceptional standards of person-centred care and support.
The service provides rehabilitation accommodation for adults with health needs and who by is recovering from trauma, surgery or a stay in a hospital setting. We are keen to recruit an individual with integrity, who can help us ensure our company values are at the heart of everything we do.

Your role will be to providing support to customers promoting and developing their life, health, and wellbeing. Contacting customer families and arranging several MDT, obtaining information and developing care plans to enable the team provide a person centred support.
Some day to day tasks may include:
Helping with preparation of meals & drinks.
Administering & recording medication
Support with high standards of nutrition
Adhere to health & safety legislation
Cleanliness & intimate personal hygiene
Provide emotional support.
These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.

A starting salary of £24,294 - £25,979 (pro rata if working less than 37 hours per week)
Designated rota pattern including daytime, evenings, weekends, and Bank Holidays
