Summary

About the Role

The New Models of Care (NMOC) and Urgent and Emergency Care (UEC) teams have been newly formed following a restructure of elements of the Commissioning Directorate.

Both teams work in partnership with colleagues in the Cheshire East Place Integrated Care Board and the Head of Service is a joint role between CEC and Cheshire East Place. The focus of the Urgent and Emergency Care team is responding to emergency and emerging situations and demands such as blockages in the health system and market failure while the New Models of Care team has a more strategic, longer term focus on market shaping, commissioning and strategy development.

Commissioning and Contract management

This team has primary responsibility for market shaping, strategy development and commissioning services from care providers such as Care at Home, Accommodation with Care and the ongoing management of those contracts.

Urgent and Emergency Care team

The Urgent and Emergency Care teamwork in partnership with Health, Social Care and Third Sector system partners across the place to manage the operational system performance of services including ownership of system risk management, capacity, system flow and lead on place-based programmes of work such as Home First:

The functions of the team are to:

Prevent unnecessary or avoidable hospital admissions by working across the community and hospitals.
Facilitate safe discharges from local hospitals in a timely manner to the most appropriate setting to meet people’s needs and maintain their independence.
Design and build a person-centred support package in partnership with the person, their strengths and support circles.
Ensure people are supported in the community, post discharge, to reduce readmissions.
Implement national guidance on discharge and drive forward the Urgent Emergency recovery plan.
Care Brokerage team

The Brokerage Service coordinates suitable support that responds at the point of need, sources planned provision to ensure we have the right amount and type of quality care and support. To provide timely access to information, advice, and informal support to prevent a hospital admission and help people to have choice and where possible to remain at home.

Key challenges for all teams are coping with increased demand and complexity of need, workforce challenges within the Health and Social Care sector and increasing costs impacting on Council budgets and ability to support a sustainable care market as required by the Care Act 2014.
About you

You will support the Project Managers in providing the smooth, efficient, and effective running of the UEC, NMOC and Brokerage teams, to maintain business continuity and achieve departmental objectives, outcomes, and deadlines. The post holder will be expected to produce high level reports, letters, documents, spreadsheets, and presentations using Microsoft Office. The Business Support Officer will support the central co-ordination, implementation and maintenance of departmental performance monitoring and internal governance, in a timely manner.

A high level of IT skill and the ability to communicate effectively at all levels and with all system partners is essential to the success of this role.

The post holder must be able to work flexibly and independently under minimal supervision and as part of a team to meet operational needs within a fast paced and high-pressure environment and to undertake duties in accordance with Council policies and procedures.
