Creative Support is a non-profit organisation which provides high quality person centred social care services for people with learning disabilities, mental health and other needs. We are currently recruiting a Neighbourhood Recovery Support Worker to work in one of the Neighbourhood Hubs and Outreach Team in Tameside, Greater Manchester.

In this role you will be based in one of the Neighbourhood Hubs working with a broad range of other professionals and stakeholders. You will respond to new referrals and assist in promoting the service to generate referrals to the service. You will assess referrals and discuss them with the Project Manager and Neighbourhood lead before commencing a focussed and time limited package of support. Support will be outcome focussed and tailored to the needs of the service user. Goals will be set and reviewed on a regular basis. Support will be provided from a strengths based model of support providing direct practical and emotional support, signposting and joint working with other agencies including community assets.

Key responsibilities would be:
• Providing strengths based, person centred support to adults experiencing a mental health or social crisis.
• Develop outcome focussed recovery support plans and risk assessments with support from the Project Manager.
• Providing direct support, therapeutic and problem solving approaches on an individual basis.
• Assisting with mental health needs, housing, welfare rights, isolation, drug and alcohol problems and other needs as identified.
• Close inter-agency working.

You will be provided with a full induction and training to enable you to successfully carry out the role. You will also be provided with regular supervision and support from your line manager and Service Manager.
