Summary
Job Summary: To be visible in your role within the practice and attend clinical and education meetings. Work with the Senior Clinical Pharmacist, Prescription Hub, clinicians and practice management. Job Responsibilities: Patient Tasks Prescription and Medication Queries Acute Medication requests Minor ailment queries Teamwork MDT Reviews Frailty medication Reviews Audits, Prescribing & Best Practice Education and Communication Point of contact for prescribing team queries Repeats and Discharges Reconciliation of Discharge Letters Supervising appropriate reauthorisations Protocols and Repeat Prescribing Protocols Formulary, Script Switch, Medication Alternatives Shared Care and high-risk drug Monitoring Any other duties appropriate to the role
