Summary


We are looking for an enthusiastic clinically experienced senior nurse who can support, lead and manage our Neighbourhood Nursing Teams including Community Matrons.
As a practising clinician and manager to ensure the provision of high quality, integrated and innovative patient centred healthcare by providing line management, professional leadership, advice and guidance to locality nursing staff. To be a key member of the operational management and development team within community nursing contributing ideas and innovative practice to moving the service forwards.
You will be responsible for the day to day operational management and clinical leadership of your area of responsibility ensuring that the teams meet expected quality standards, objectives, KPI's and maintain their clinical expertise.
You will be required to be flexible and adaptable and work across different sites and teams as needed with an ability to demonstrate a creative approach to solving problems and empowering teams.
This role involves active clinical practice alongside management and visible leadership within teams.

You will be responsible for the day to day operational management and clinical leadership of the area of responsibility. Duties fall into the following key areas: Performance management, risk management and clinical governance, professional leadership and staff management, department and staff organisation, strategy and service improvement, financial management, research, education and training and professional standards and patient experience.

Professional Leadership and Management

Clinical Governance
• Understand national and trust policies and procedures, supporting and coaching teams in the implementation and processes involved in following them. Lead the teams in providing the assurance needed at service, directorate and organisational level that teams are providing safe and effective care, the service is meeting it’s KPI’s safety and quality standards;
• Work jointly with business support to monitor necessary reports and databases to ensure robust clinical governance is in place. Working with the teams to ensure a robust clinical governance and assurance systems are in place in all areas to evidence and demonstrate KPI’s and quality and performance standards;
• Discuss with teams the data provided to them by business support to ensure productivity and efficiency is at its highest;
• Ensure that the team is meeting its data quality and performance and quality indicators through monitoring, support, data collection and facilitating quality team discussions;
• Provide communication to the teams where relevant from the trust to ensure teams are aware of trust initiatives, policy changes and important events;
• To ensure the team undertakes benchmarking, audit, research and evaluation as required ensuring that they undertake and implement activity relating to improving care and that the data is submitted on time;
• Facilitate discussions within teams to ensure patient safety and robust safeguarding processes and procedures are in place.

Clinical
• Must be an expert practitioner and able to support the teams with clinical guidance;
• Support the teams with complex patient discussions to ensure safety and best practice is maintained for the patients;
Undertake at least a half day a week in face to face clinical practice;
• Ensure that teams and individuals are competent to meet the full needs of patients and to deliver evidence-based practice;
• Ensure that teams follow clinical policies within the trust, facilitate team discussions to identify what can be locally adapted;
• Support the team when clinical escalation occurs and liaise with the Clinical Nurse lead to ensure patient safety is paramount.

Service Development
• Support the trust in the development of a Neighbourhood Nursing model liaising with stakeholders and management within the trust to increase awareness and assist in making improvements to the model;
• Provide and support external trusts and departments who are interested in the model;
• Support and coach the teams in change management and encourage ideas to be discussed;
• To contribute to the business planning, process and support the transformation of teams to a Neighbourhood Nursing model by contributing to the workforce planning, service improvement and development.

Education, Training and Mentorship
• Participate in relevant Neighbourhood Nursing training where necessary and relevant, support other coaches in this field;
• Participate in a network of coaches, internal and external, to offer and receive support within the Neighbourhood Nursing model and general coaching;
• Undertake any relevant coaching training or events within the trust;
• Provide support or training to external departments or areas requiring Neighbourhood Nursing knowledge or training;
• Liaise with the Neighbourhood Nursing teams to ensure GSTT NN frameworks are understood and followed;
• Liaise with the teams to ensure they are aware of how to access training and development within the trust and their responsibilities. Monitor mandatory training and ensure teams are meeting all PDR and mandatory training requirements;
• Support team members with the implementation of the career framework, when required, for nurses to progress through agenda for change spine points;
• Ensure continuing, personal and professional development, taking ownership and accountability for staying up-to-date.
