Summary
Leadership and Service Development: Work strategically with the Executive Team and Provider Collaborative Partnership, statutory and third sector organisations, commissioners and PCNs to integrate the service pathway within the health and social care system. To promote and encourage service-user involvement and co-production in all aspects of the service. To support Provider Collaborative partners to identify gaps in provision in the health and care system, flagging up these gaps to senior managers and commissioners by means of agreed communication channels. To develop robust and strong relationships with ethnic minority groups and people with protected characteristics, including informal family carers, within each of the Sandwell six towns, embracing an inclusive approach to developing resilient and strong communities. To work with the Operations and Development Executive and partnership senior managers to deliver the service contract KPIs. Develop partnerships with PCN/GP leads, Mental Health Trust Adult Social Care and acute services leads to promote SCDS, evaluating the effectiveness of the service reach and integration of delivery. Operational responsibilities: To manage the service as a whole working closely with the partnership team to deliver the full-service specification. To support the policy, procedure and practice for all staff employed within this service and support lead partners and their staff, taking a whole system approach. To work in conjunction with all partners ensuring there is clear and good communication with internal colleagues, partners and service team members to deliver a quality service and to meet contractual requirements. To be responsible for holding regular meetings with team and partners to ensure contract compliance KPI returns are returned by the required deadlines. To create effective channel of communication with internal & external stakeholders, including Black Country Healthcare NHS Foundation Trust, Sandwell and West Birmingham NHS Trust, iCares, STARS, Sandwell Metropolitan Council, GP Practices/PCNs. To provide accurate KPI reporting and analysis of data to senior managers and commissioners to meet the compliance requirements for this contract. To ensure the CRM system is capturing the relevant data to support KPI evidence. To provide management guidance and support (with partner organisations) and clinical supervision for Dementia Navigators and Community Dementia Advisers, to safeguard the wellbeing of staff and address development needs of the team. To support training arrangements to ensure skills set and knowledge of staff working within this service. Making sure screening tools and procedures are fully understood, and staff have the knowledge and understanding to support service users with legal, financial issues to include LPAs and Advance Care Planning. To check staff compliance with the use of Charity Log CRM software system to ensure accuracy and timely recording to meet contract activity reporting. To be a designated safeguarding lead, responsible for managing and embedding safeguarding practice within the partnership, ensuring internal and local safeguarding procedures are followed, and addressing identified issues to ensure vulnerable people are protected and harm is prevented. To work with the Operations and Development Executive and service users in the co-development if the service - ensuring we have focus groups and events, as well as regular client feedback, regular client surveys and focus groups. Marketing and Promotion: To promote the service as required by the Operations and Development Executive, presenting the service to Health and Social Care professionals. To ensure that the marketing and promotion of the service functionality is supported and embedded within the collaborative partnership. To ensure that there is a standard message that is easily understood and enables all potential service users and those likely to refer into the service to understand the pathway and how people with a memory problem will benefit from being supported by the service. To lead on the development of raising the awareness of Dementia Friendly Communities to the community, education settings and other organisations within Sandwell. To actively help and support leads within each of the six towns to have local resources and service awareness to provide appropriate access to information and advice to provide to service users, building knowledge and resources and increasing awareness of dementia within local organisations to support service users through their journey of dementia.
