Summary


Dorset Healthcare are looking for an Urgent Community Response Support Nurse to join our supportive team based here at Wareham, covering the Purbeck area.

The multi-disciplinary team work with adult patients in their own homes or care homes to promote independence and improve quality of life by supporting them to remain at home. The post of Urgent Community Response Support Nurse has been funded to enhance the ability to avoid unnecessary admissions to hospital and support timely discharges from the acute and Community Hospitals. Dorset are also developing Virtual Wards to provide clinical care to patients with acute needs within their own home, of which this role will be very much involved.

The role of Urgent Community Response Support Nurse will be required to provide clinical assessment of patients in crisis and be able to problem solve and make decisions about their treatment. The role will also provide follow-up monitoring and assessment of these patients on your own caseload. We offer opportunities for further training and development. There is also the chance to be involved in Service development projects.

This role is a full time post however part time would also be considered of a minimum of 22.5 hours and as long as the services needs are met. Hours covered by the service are 08:00-18:00 7 days per week with weekend enhanced pay!

Undertaking clinical observations and nursing assessments, acting on findings and liaising with the MDT as required

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
