Summary

The post holder will be advocating and working with adults with complex needs and their carers, working collaboratively with colleagues from other agencies to develop support and services appropriate to peoples individual needs utilising a strength based approach. The remit of the service is to support the citizens of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and/or mental health needs. The post holder will need to be confident carrying out outcome-focused assessments, with support planning, to meet their care and support needs. The post holder must be able to work with a diverse group of individuals and with our partner agencies to deliver the best outcomes for our citizens. The Trust is committed to safeguarding and promoting the welfare of children and young people/vulnerable adults and expects all staff to share this commitment. As a Trust, we offer informal support and advice as required, monitored caseload numbers, monthly formal supervisions and membership to the Social Work Learning Academy, which supports ongoing professional development opportunities, learning and career development. You will have direct support from the Practice Lead and colleagues as well providing peer support to your team. You will need to be able to travel across the locality. This is an interesting and challenging post for someone who has a commitment to improving outcomes for the citizens of Staffordshire. Social Work Learning Academy (SWLA) On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
