Summary

Utilise available data and quality improvement strategy and ensure this process runs efficiently by : Running monthly reports for individual QOF domains and contact the patients using SMS, telephone, email or letter to ensure contact is made within the correct time scales and the correct read codes are used for reporting purposes Running a monthly report for all enhanced services, including high risk drug monitoring recalling patients as above. Running a monthly report and inviting patients in for their NHS health check. Running monthly CQC searches, completing any action required and updating the monitoring spreadsheet, highlighting any issues to the senior management team. Supporting the Practice manager in the submission of month end/year end data. Organising and recalling of eligible patients for vaccination clinics/home visits. Assist and undertake clinical and administrative audits Check and correct patient coding against business rules for GP contracts i.e: QOF/ QI/ IIF/ QOEST Work with clinical and non-clinical staff to improve the accuracy of data input Assist with maintaining accurate disease and QoF registers within the clinical system Proactively seek and implement a variety of quality and improvement projects, including those directed by the senior leadership team including review significant events and complaints processes in line with Practice policy.
