Summary


The Specialist Dementia and frailty teams provide a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 70 years of age, who have functional mental health needs with an associated age related frailty.

You will be responsible forassisting with the delivery of patient centred care under the supervision of a senior member of the team; providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the team.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, workingshifts where appropriate and operationally required.

The Specialist dementia and frailty teams are community based and it is essential that you are able to drive a car and have a vehicle for work purposes with valid UK driving license.

Please not that this position is a 6 month fixed term contract of 22.5 hours a week (3 days) to cover maternity leave.

You will be working as a community support worker to assist the multidisciplinary clinical team, working in one specified pathway or across pathways in accordance with area / locality structures, You will be working with Service users in the community in their own homes or place of residence across the MID locality covering Chelmsford, Maldon and Braintree. Your role will include providing physical health care checks, emotional support, carers support and signposting to other organizations.
