Summary
Undertake comprehensive clinical assessment of the complex health and psycho-social care needs of patients with diabetes Establish individuals functional capabilities with regard to diabetes clinical management. Plan, implement, monitor and review therapeutic interventions with individuals who have diabetes and their carers. Investigate and diagnose the unwell individual with diabetes Lead complex medicine management review and prescribe medicine and appliances for patients using independent prescribing skills. Lead on the development or review of clinical policies and guidelines related to the care of patients with specific disease / conditions within the business unit. Take a Leadership role in the continuous development of the service.
