Summary


You will be a member of a small & friendly team based on the Hillingdon hospital site.

The job involves providing support to service users and their carers in their homes, residential care homes or nursing homes. This may involve supporting people to attend social or therapeutic activities in the community.

The service also supports the older adult mental health wards to facilitate discharge and where possible to avoid admissions by providing support and treatment at home.

Work under the direction and supervision of qualified staff to assist in the assessment of patient needs.

Implement individual care plans, which are designed to assist each patient in the promotion of his or her mental and physical health.

Carry out procedures considered appropriate by the qualified staff with minimal supervision.
Take part in, and help patients participate in occupational, diversional and recreational activities

Ensure patients make informed decisions whenever possible based on open and honest dialogue with the patient and their Carer, and, ensure patient’s choices are respected where possible.

Build and maintain sound therapeutic relationships with patients and their families/carers.

Record all interventions as directed and supervised by qualified staff.
Maintain liaison with and participate in the inter-disciplinary and multi-professional approach to care.



An exciting opportunity has arisen for a highly motivated and enthusiastic Band 3 Support Worker to join our Older Adults Community Mental Health Home Treatment Service.

The Home Treatment Service (HTS) works with Older adults covering the Hillingdon borough and is based in Woodland Centre in Hillingdon.

The HTS is a needs-led service and work with service users that are physically frail and have a mental health problem. Caseloads are a mixture of service users with long and enduring mental health issues, acute mental health issues or issues relating to memory loss. It is an exciting time to join the team when services are changing to be more dynamic and responsive to the service users within the Borough.

The HTS is a multidisciplinary team, supported by a good administrative system and offers an excellent work environment. The team has good partnership working with local non-statutory Dementia services. The team works closely with our colleagues at The Woodland Centre at Hillingdon Hospital; Oaktree ward, Memory Services and Older Adult Community Mental Health Team.

PERFORMANCE STANDARD

Act as a direct care giver for a given group of patients within an identified framework and model of care.



PERFORMANCE INDICATOR

1.1 Work under the direction and supervision of qualified staff.



1.2 Assist in the assessment of patient needs.



1.3 Implement individual care plans, which are designed to assist each patient in the promotion of his or her mental and physical health.



1.4 Carry out procedures considered appropriate by the qualified staff with minimal supervision.



1.5 Take part in, and help patients participate in occupational, diversional and recreational activities



1.6 Escort patients to various therapeutic/treatment programmes as determined by their care plan and the clinician in charge.



1.7 To act in a manner to raise standards of care and make the services culturally sensitive, relevant and accessible to meet the needs of the patients.


• Ensure patients make informed decisions whenever possible based on open and honest dialogue with the patient and their Carer, and, ensure patient’s choices are respected where possible.


• Build and maintain sound therapeutic relationships with patients and their families/carers.


• Record all interventions as directed and supervised by qualified staff.

• Co-operate with, contribute to and enact all aspects of daily work as co-ordinated and planned for by qualified staff.



2.3 Ensure that agreed Trust policies and procedures are followed.



2.4 Contribute to the setting and maintenance of standard setting and use audit systems.



2.5 Liaise with colleagues, finding ways to optimise and improve services without increasing costs.


• Adhere to Sickness and Absence policy.


• To support qualified staff with administrative tasks as required.


• Maintain a diary of appointments to include home visits, clinical and administrative meetings, and daily mileage/travel and submit to Community Mental Health Team Manager as required.


• To fully participate in team rotas to ensure service delivery.

Ensure open channels of communication with both internal and external stakeholders



PERFORMANCE INDICATOR

3.1 Liaises with community services and carers regarding patient care.



3.2 Maintain strict confidentiality of patient information at all times.



3.4 Report to senior staff on matters of difficulty.


• Take part in /team briefings/meetings as directed by CMHT Manager.


• Keep up-to-date of developments within the service and Directorate by reading circulars, memos and information.



3.7 Accurately document all relevant communication with colleagues and team members.





4 KEY RESULT AREA:- PERSONAL DEVELOPMENT



PERFORMANCE STANDARD

Promotion of a positive image of the service through self-development.



PERFORMANCE INDICATOR

4.1 To participate in supervision and appraisal processes to enable reflective practice and person development.



4.2 Participate in in-service training considered appropriate and participate in other training as appropriate.



4.3 Participate in special projects, research and surveys related to the clinical area as directed.





5 KEY RESULT AREA:- HEALTH AND SAFETY



PERFORMANCE STANDARD

Ensure that the service is safe as laid down in the Health and Safety Act 1974, Trust Health and Safety policy manual and local Operational Policy.





PERFORMANCE INDICATOR



5.1 Understand and carry out responsibilities by adherence to the guidelines laid down in Health and Safety, COSHH, Fire Training and local policies directly affecting patient and personnel.



5.2 Report to the HTS Team Leaderon all matters relating to health and safety issues.


• To report incidents and accidents in line with Trust policy.
