Enhanced DBS disclosure required
Motivated Grade 8 Social Worker’s to join the Hospital Social Work Team. Work with Adult’s over 18 to facilitate a safe transfer of care from hospital and then follow them into the community to assess long-term care needs and maximise their independence. You must enjoy working in a fast-paced environment and working closely with multi-disciplinary system partners.
Key Responsibilities


Manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, and being part of a duty rota.
Deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible.
Enthusiastic to engage in adopting strengths based, person centred assessments.
Engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks.
Predominantly be supporting with hospital discharges at either sites at Pinderfields or Dewsbury District Hospitals.

Qualifications:


DipSW or equivalent
Registration with the HCPC

Knowledge:


Understanding of partnership working.
Working with diversity.
Relevant legislation and standards

Experience:






Substantial relevant post qualifying experience.
Partnership working across agencies.

Skills & Abilities:


Keyboarding skills are required to ensure accurate and appropriate information is produced and provided to service users, customers and clients.
