Summary

Home First supports the approach of “why not home, today?” Wherever possible patients should be supported at home for ongoing assessments where this is not possible alternative pathways are available. Staying in the hospital longer than necessary hurts patient outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term well-being (www.england.nhs.uk). Working across organizational boundaries and as part of Surrey Downs, Home First is a multidisciplinary and interagency service leading on and supporting discharges from hospital for those patients with ongoing health and/or social care needs by identifying the most appropriate pathway, allocating team members to work on the discharge and ensuring discharge and follow up assessments take place in the community on time.


Home First hub coordinates the process from assessment in the hospital (Acute and Community) to discharge into the community with relevant service involvement. This includes but is not limited to Therapy (SDHC @home team and PCNs, Care (Reablement and Brokerage), Bed-based rehabilitation, short and long-term placement. Home First Clinician of the Day is responsible for triaging, leading on complex discharges, supporting the wider team, ensuring all systems are updated and ongoing service developments keeping the patient at the centre.
The service is operational 7 days a week with clinical and administrative cover across this time.

To lead on triaging decisions on the day to support and manage the Home First+ pathways

To assess and identify the needs of clients with a variety of complex social and health care needs and support their hospital discharge with the appropriate support in the community
To work with the Home First Clinical Lead to support the roll out of Home First + within other areas e.g. Community Hospitals
To participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population
To promote effective teamwork within the wider Surrey Downs Health and Care / Sutton Health and Care, delegating activities to the wider team to the benefit of the patient and service delivery
The expectations within this job description will be achieved through virtual and hands-on clinical practice, service development, education, training and research
To support @Home service with provision of appropriate nursing and therapeutic care in the community
To work as a key member of the multidisciplinary team in the assessment and management of a caseload of clients
To support @home COD by deputising if and when required with referral taking and triage for @home team.
To undertake clinical work if and when required based on the needs of the service
