Summary
It’s an exciting time for our Hospital at Home Service. We are expanding and recruiting to our new teamswho will work with patients in their homes to provide a virtual ward service for early supported discharge and admission avoidance.
Hospital at Home teams will provide specialty virtual ward beds. They will be multi-speciality supporting patients across respiratory, heart failure and frailty in partnership with acute care, consultants and specialist nurses.

This is an amazing opportunity to become a member of the team working together to support patients through the critical first few days after they leave hospital and avoid unnecessary hospital admissions.
We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm.
The team works closely with other services within Sussex Community NHS Foundation Trust, as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations.
You will deliver care within your proven competency as achieved during your Nursing Associate Training, and contribute to the planning, implementation and monitoring of patient care as part of the Community Nursing Team. You will recognise changes in a patient’s condition and seek assistance from a Registered Nurse of band 5 or above and/or GP as appropriate.
If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support.
You will need to able to drive with a full UK licence with access to a car due to the geographic area covered by the team.

As a Nurse Associate, you will have completed, or bein the process of completing, an NMC-approved Nursing Associate programme. You will also be registered with the NMC as a Nursing Associate or bein the process of registering.
