Summary


Are you motivated, enthusiastic and committed to working in a recovery focussed approach? Bridport Older Persons CMHT is looking for a Community Mental Health Nurse to join their team!

As a Community Mental Health Nurse you will be a dedicated care coordinator responsible for a caseload of older adults in the community with severe mental illness who have multiple and complex needs. You will provide a comprehensive accessible and responsive mental health service to Older Persons and their Carers within a defined locality. You will work alongside colleagues as part of a cohesive team in assessing people in community settings, delivering personalised well- being care plans, undertake risk assessments and provide high quality care and treatment in conjunction with other professionals.

The Community Mental Health Nurse is 37.5 hours position working Monday to Friday 9 to 5. You will be based at Bridport Community Hospital and will need to have a valid driving licence and use of a car to travel to assess and monitor patients in their own homes and within other community or inpatient settings.

To support the team and locality leader in implementation of the DHUFT Mental Health Strategy.

To continually develop clinical, team and service evaluation and delivery of service.

To contribute to effective multidisciplinary working within the CMHT and to build up effective liaison with community, primary and other secondary and tertiary services and agencies within the locality to provide a holistic care package based on an individuals needs and service development
