Summary

We are looking to appoint a Nurse Practitioner to work as a member of the Hospital at Night Team at Shrewsbury and Telford Hospitals NHS Trust. The team works in the out-of-hours period, covering all wards across the Medical Division alongside the on-call Medical Team.

The post holder will hold a physical assessment course and a Non-Medical Prescribing qualification at degree or Masters Level.

Those that apply will have current and up-to-date experience of working in Acute Medicine as a Nurse Practitioner, be able to work autonomously and possess excellent communication and leadership skills.

Whilst the emphasis of this role mainly focusses on the clinical management of acutely unwell deteriorating patients, there is also an expectation that practitioners will be involved in delivering teaching to support the development of new trainees within the service, as well as educating junior staff on the wards.

If you feel that you have the range of clinical skills necessary to regularly review sick patients as well as the non-clinical attributes that ensure quality of care and support of your junior colleagues in contributing to patient safety, we would like to hear from you.
• The principle role will be to provide professional, advanced clinical support and advice to the junior doctors, nursing staff and other members of the multidisciplinary team working within the Trust
• The post holder will hold advanced physical assessment skills in order to assess patients primarily on the inpatient wards.
• The post holder will determine an appropriate course of action
based on their assessment to prevent clinical deterioration of
the referred patient
• The post holder will be able to identify a deteriorating patient as
a priority in their workload
• The post holder will be aware of the need to escalate, and
facilitate a patient to the appropriate level of care
• The post holder will recognise the need to be flexible working
across a trust which is based on two separate site

As an organisation we strive to provide high quality, safe care for our patients in an environment which our staff are proud to work in. We believe that by adhering to our Vision (“To provide excellent care for the communities we serve”) and working with our Trust Values in mind (Partnering, Ambitious, Caring, Trusted) we can achieve the right results for the people that matter most – our patients and their families.

The Hospital at Night Team currently covers a 7 night a week service cross-site with allocated time for ongoing development and clinical supervision. We are an integral part of the Trust's Cardiac Arrest team and respond to all calls together with the Medical Team. All Nurse Practitioners are ALS Providers or working towards this qualification.

This is an exciting opportunity to work alongside a team of practitioners who are striving to improve the quality and safety of the care delivered in the out-of-hours period. The successful candidate will be able to respond to deteriorating patient calls adeptly and will also act as a role model to others.
