Summary


We are looking for an Advanced Clinical Pracitioner to join our forward thinking and supportive team at Health E1 medical Centre where you will be provided with an exciting opportunity to practice and further develop your clinical skills.

Health E1 is a general practice managed by the East London Foundation Trust (ELFT). It registers patients who are homeless (this encompasses street homeless, hostel dwellers, those in temporary accommodation and those who have no secure housing). Patients register with the practice and are offered a full range of primary care services. We are a teaching practice and regularly have students on placement. We are part of PCN Network 2, which is a longstanding well-established network, and we work closely with our network colleagues.
• The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice
• The post holder will work collaboratively with the multi-disciplinary general practice team at HE1 to meet the needs of patients, supporting the delivery of policy and procedures.
• The post holder will build effective working relationships with other health care professionals.
• The post holder will work with the senior lead clinician and the other senior leads including lead GP and Practice Manager.
• The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.
