Summary


The post holder will work closely with the Clinical Nurse Specialist (CNS) team, infectious diseases Consultant and other members of the team. They will be required to deliver an outreach service to Hull, East and North Yorkshire, North Lincolnshire and Goole (NLAG) with the use of the outreach van. You will require daily travel driving the outreach van in Hull, East Riding, NLAG and occasionally North Yorkshire.

Working as part of the Hepatitis team you will support the CNS to promote testing and treatment for Hepatitis C. Working flexibly the post holder will ensure that they provide a patient-focused service at all times. Flexible working hours will be necessary to ensure patients will be seen at suitable times for their needs. This may require long days, early morning and evening work. We are seeking someone passionate about making a difference to people living with Hepatitis C assisting with the acceptance of a new diagnosis and treatment. You will need to possess clinical skills, excellent communication and interpersonal skills. The post holder will undertake a range of duties to ensure the effective delivery of care with support from the CNS. Experience caring for patients with a history of substance misuse is desirable but not essential. A valid UK driving license is essential

Working under the guidance of the Lead Hepatitis Specialist nurse and the Hepatitis team the postholder will implement care packages which will ensure a smooth pathway for patients with Hepatitis C from diagnosis through to treatment.

To help towards NHSE Hepatitis C elimination programme the postholder will work in a hospital and community setting assisting with the increase of referrals, screening, completion of treatment. They will work as part of the team as well as independently, covering Hull, East and North Yorkshire, North Lincolnshire and Goole.

The post holder be responsible for implementing nursing care programmes e.g. oral swab, dried blood spot sampling, and venepuncture blood sample collection in line with Trust procedures as requested by the medical/nursing staff.

The post holder will also be responsible for the fibroscanning of patients following referral by the multi-disciplinary team.

The post holder will perform tests such as dried blood spot, blood sample collection and fibroscanning and will participate in multi-disciplinary team meetings to support the MDT
The majority of work will be based on the Hepatitis C outreach van with a support worker.
