Summary
To provide high quality data entry and administration support to the Stroke Service through the use of the Sentinel Stroke National Audit Programme (SSNAP) web tool, ensuring information is collected and inputted accurately and attention to detail is maintained at all times. An element of ward-based working will be necessary as there is a key requirement to enter data from the start of the patient journey through to discharge from hospital.To collate relevant information from case notes and clinic electronic systems for all stroke patients attending the University Hospitals of Leicester (UHL). To accurately compile and input this information onto the database in a timely manner, ensuring data completeness on a monthly basis.To liaise with and seek advice from medical, nursing and administrative staff where necessary, ensuring accurate data entry is maintained at all times.To ensure all missing data is retrieved.To liaise with other stroke service members regarding data issues relevant to this role.To provide cross cover for the database administrators.To use hospital operating systems and other related support systems to ensure all data is captured accurately and swiftly.To liase with Medical Records to ensure patients case notes are requested and tracked as required by the data team. To ensure the integrity of data quality is maintained.To work flexibly to meet the needs of the service.To undertaker data audit and regular monitoring of data as required.To attend Stroke service meetings internal and external to the Trust as required.To maintain a polite and helpful telephone manner, observing the need for discretion and courtesy at all times.To ensure confidentiality is maintained at all times and to be aware of Trust guidelines. To be aware of the Data Protection Act and take appropriate care of patient information.To contribute to development of a co-operative and supportive team environment.To identify areas of improvement in line with SSNAP guidelines providing feedback to the Team Leader as required.Any other duties as may be deemed necessary from time to time.
