Summary


London North West Healthcare NHS Trust is currently expanding the Bank Service. We are now looking to recruit experienced, flexible and reliable Healthcare Assistant to help play a key role in providing flexible cover within our Trust.

Applicants should be self-motivated, responsible, and empathetic to the individual needs of patients and be able to demonstrate a high level of commitment to patient care. You must also possess excellent communication skills and be open to new ideas and ways of working.

 To work as a member of a multidisciplinary team assisting in the provision of
care, support and rehabilitation to adults requiring intermediate care.
 Responsible for the care of the patients on the ward.
 Delivery of Quality Patient Care.
 Progression of Patient Rehabilitation.
 To be responsible for the care of a group of patients during the shift.
 Assists in meeting the requirements of the Clinical Governance programme.

Clinical Duties
 To undertake tasks in delivering patient care and rehabilitation as directed
by service professionals
 To progress patient treatment, following initial assessment and care
planning by an appropriate service professional. To observe any changes
that affects the individual and accurately communicate and document
these back to the appropriate team member.
 To take responsibility for the well-being and comfort of patients. To
observe an individual’s ability to tolerate and manage their treatment
programme. To alter, adapt or stop activity in response to the individual’s
ability and state of health.
 To undertake physical measurement monitoring as directed (e.g. blood
pressure, oxygen saturation, heart rate etc) and report findings to the
appropriate service professional.
 To communicate effectively and professionally at all times with patients,
carers, service professionals and other health and social care colleagues
as appropriate.

 To reflect on and evaluate one’s own practice in order to improve the
delivery of patient care.
 To ensure tasks, therapy and care are only carried out within personal
competency and that personal competency is maintained and developed.
 To be responsible for organising, prioritising and planning own workload to
meet patient and service priorities, readjusting plans as situations change
or develop.
Clerical Duties
 To share responsibility with all team members for the day-to-day smooth
running of the service, making specific contributions as requested.
 To participate and assist with service audits as required.
 To record and collate statistics as required.
 To maintain clear, comprehensive and accurate clinical records and to
manage all documentation processes (e.g. reporting of clinical incidents /
near misses).
Other Responsibilities
 To undertake any other such duties as may be required from time to time,
that are consistent with the responsibilities of the grade.
 To comply with the Trusts and departments policies, guidelines and
procedures.
 To attend all mandatory training as required in line with Trust employment
and professional need.
 To actively participate in team meetings, adding value to the department’s
business planning, service developments and clinical governance
initiatives.
 To participate in performance and development programme and to be
committed to learning. To participate in clinical supervision and individual
performance development process during routine work time.
General
 The nature of this post is one of continual development and the duties and
responsibilities outlined above may change from time to time to reflect the
changing needs of the organisation and people’s changes in
circumstances. The post holders will be expected to comply with the Trusts
quality standards at all times
