Summary

Job summary: The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role includes the delivery of evidence-based practice for patients presenting with chronic disease management, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required. Main duties and Responsibilities: To assess, plan, implement and evaluate clinical nursing care to provide services in; Treatment room. Cervical smears. All aspects of Ear care. Health promotion. Deliver care in chronic disease management. Flexibility to take on extra hours / change shifts during annual leave or during periods of absence as required or event of pandemic. Maintains high standards of infection control at all times thus protecting patients, self and colleagues. Actively engages patients and carers in the management of their health, promoting health and wellbeing. Refers patients to other members of the practice team in line with agreed protocols. Maintains accurate records of all clinical activity in line with practice protocols and standards. To triage patients over the phone to the appropriate member of clinical staff with the appropriate skills to meet a patients stated needs. Contributes to clinical governance activities within the team including audit and evaluation and seeks to underpin practice with current evidence of effectiveness. Supports members of the practice team to maintain health, safety and security. Contributes to teaching and offers support to fellow colleagues within the practice team. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Uses appraisal and supervision to identify own development needs and maintains and develops competence in line with agreed requirements. Stock control / stock rooms / keeps up with health promotion displays / activity and other administration duties as deemed appropriate. Any other duties as required. Full training will be provided in all areas as required. Health & Safety: The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: Using personal security systems within the workplace according to practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Reporting potential risks identified. Contribution to the Implementation of Services: The post-holder will: Apply practice policies, standards and guidance Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audit where appropriate.
