Summary

Support Worker | Warrington | 37.5 hours per week | Rolling shift pattern – 4 days on, 4 days off – 08.30am – 20.00pm | £25,125.00 - £28,318.92

For over 50 years, our client has provided safe, high-quality accommodation and support services to the most vulnerable members of the community. Their mission is to offer person-centred care and support across Wirral, Cheshire, and Warrington, with a focus on individuals aged 18 and over who are at risk of homelessness or are care leavers aged 16 to 21.

As a trusted partner of local authorities, health organisations, and community sectors, they are committed to delivering excellent outcomes for over 500 individuals each year through their outreach floating support, supported housing, and supported lodgings programs.

Are you the right person for the job?

• NVQ Level 2 or equivalent in Health and Social Care
• Strong verbal and written communication skills
• Observant, IT literate, and able to follow procedures
• Experience working with vulnerable groups, managing caseloads, and reporting
• Understanding of health and safety issues and excellent interpersonal skills

What will your role look like?

As a Support Worker, you will play a vital role in providing high-quality housing-related support and services to individuals with diverse needs, including mental health issues, substance misuse, and complex needs. Your responsibilities will include:

Case Management: Developing personalised plans to guide service users towards independent living, while ensuring accurate documentation and compliance with policies
Caseload Management: Supporting and coaching service users across various areas such as accommodation, health, education, and employment to achieve sustainable outcomes
Partnership Promotion: Building partnerships with agencies and departments, facilitating referrals, and promoting community engagement to meet service users' aspirations
Housing Management: Assisting in rental income collection, debt management, and maintaining exemplary community relations
General Duties: Participating in training, maintaining confidentiality, adhering to safeguarding frameworks, and upholding equality and diversity principles
