Summary
Lead, develop and oversee the clinical pharmacy service provided to the Urgent Response and NHS@Home services Responsibility for leading pharmacy input into the Urgent Response and NHS@Home service at the Great Western Hospitals NHS Foundation Trust Support and address a range of pharmaceutical care needs for patients, such as prescribing or deprescribing, medicines reconciliation, medication review, medicines adherence assessments, ensuring timely medicine supply, antimicrobial stewardship, and therapeutic drug monitoring for patients within these services Support the delivery of other domiciliary medicines optimisation services as new care models emerge and participates in the delivery of other community clinical pharmacy services as required To deputise and support the Lead Pharmacist for Integrated Community Services Please see the attached job description for full information.
