Summary

Health Improvement Lead. Full Time, Permanent.
Based in Exeter please note, your base will be one of our 3 hubs, determined on your current location.

We have an exciting opportunity to join the Together team based in Devon, as a Health Improvement Lead.

We’re passionate about supporting individuals who face complex issues, including substance misuse, mental ill health, and other harmful, addictive and offending behaviour, to improve their lives.

The Lead will focus on Health Improvement and liaise with a range of partners to support this. This includes learning from deaths (liaising with Drug Related Death Co-ordinator and Domestic Homicide Co-ordinator), leading on a new approach to GP Shared Care (liaising with GPs and Practice staff), and looking at development of mental health pathways.

The Health Improvement Lead will also focus on harm minimisation across the county, encouraging safer practices that reduce the harm that people do to themselves or others from their substance use. This will include working towards Hepatitis C Elimination programme and overseeing Needle and Syringe Provision and will involve working closely with the Clinical Team as well as external partners.
As a Team Lead, you will have some line management responsibilities across the Devon area and will oversee the Hospital Liaison Workers to help support the pathways from Hospital to the community.
