Summary


We are seeking a meticulous and empathetic individual to become a Care Plan Coordinator at our residential care home. Do you excel in fostering good relationships with families and healthcare professionals to ensure holistic support for those in need?

We are an independent local charity caring for older people and people living with dementia in our nine residential care homes across Sheffield. Our Care Plan Coordinators collaborate with residents, their families, team members and interdisciplinary teams to produce comprehensive care plans that promote independence, dignity and wellbeing for our residents. You will undertake regular reviews in response to evolving needs and changing circumstances, acting as a liaison between individuals and providing valuable administrative support to the Care Support Workers.
