Summary
The post holder will- Act as a role model in the field of clinical pharmacy and provide a consistently high level of pharmaceutical advice and support in line with corporate objectives. Work on a multidisciplinary level with health and social care professionals in both primary and secondary care to deliver patient focussed care in the community, ensuring that the individual patients medicines support needs are met. Support nursing and care homes across East Lancashire to meet the required standards for the safe, secure handling and administration of medication. Carry out medicines assessments to identify inappropriate or problematic polypharmacy. Support the safe transfer of care by undertaking medicines reconciliation.
