Summary

Job summary PRACTICE NURSE The post holder is responsible for the delivery of basic practice nursing services care to the practice population. Supported by other practice nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Key responsibilitiesClinical Care synopsis:Work alongside other clinicians in the primary care team in the provision of patient care. Undertake and record the following: Complex wound management Health promotion and screening Vaccination and Immunisations Ear care Emergency management Child health/Child protection Mental Health Childhood immunisations Travel health advice and immunisation Measurement of height and weight Urinalysis Measurement of blood pressure, temperature and pulse Recording of ECGs Venepuncture Adherence and proactive to the demands of the Quality and Outcomes Framework with particular view to Diabetes, CHD, Asthma, COPD etc. Diabetes1. Initiate the process of diagnosis on patients with suspected diabetes2. Manage patient newly diagnosed with diabetes under protocols3. Undertake annual review of patients understanding and ability to self manage 4. Refer to dietician, chiropodist and diabetes and specialist nurses5. Audit outcomes of care against standards and initiate changes as necessary Cardiovascular Disease1. work with patients to set action plans for changes of lifestyle2. work within agreed protocols when managing risk factors3. to enable patients to manage their own disease4. provide psychological support by discussing interventions and treatments Respiratory Care:1. Manage newly diagnosed patients with asthma and COPD2. Work within agreed local/BTS guidelines, use of spirometry and patient group directions to manage patients with asthma, aiding diagnosis, reviewing inhaler technique and advising on treatment changes. Family Planning and Womens health:Provide a comprehensive family planning service and sexual health service to patients in conjunction with the nursing team and GP lead. This would include all aspects including coil fitting Male Health:Initiate opportunistic consultations with patients to cover issues of mens health; make appropriate referrals General:Be aware of current evidence based approaches to patient care and demonstrate this in practice.Demonstrate clinical expertise in practice.To undertake a recognised training programme leading to any appropriate qualification specified by the Practice Manager.To undertake work away from the surgery where necessary. Visiting patients in their own homes or at other centres in the practice area, travelling in a timely fashion.Assist clinicians in emergency management of patientsAct as chaperoneRecord all contacts with patientsEnsure stock items are ordered and provided in GP and treatment roomsMaintain clinical equipment efficiently and ensure servicing is undertakenFollow infection control standardsContribute new ideas to improve patient care and nursing work practicesTo be familiar with and operate with practice based formulary, computer systems and record keepingTo ensure that all work carried out, meets required quality standardsTo develop and maintain good relationships with colleagues and patients and to work in a professional mannerTo comply with Health and Safety legislation and guidanceTake appropriate action if standards are not being metRecognise potential areas of conflict, preventing where possible and report to senior members of staff if unable to resolveMaintain the highest standard of patient and staff confidentialityUndertake other tasks as may be requested by the Practice Manager or GPs from time to time, which are within the Practice Nurses competenceApplicants will be asked to undertake a Criminal Records Bureau checkApplicants may be asked to attend a pre-employment occupational health assessmentProvide guidance and training to Health Care Assistances and other Practice Nurses where appropriate.Clinical practice Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being Implement and evaluate individual treatment plans for patients with a known long-term condition Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care Deliver opportunistic health promotion using opportunities such as new-patient medicals Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care Assess and care for patients presenting with uncomplicated and complicated wounds and their management Support and advise women requesting information relating to family planning needs Support and manage health needs of women presenting for cervical cytology consultations Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children Advise, support and, where appropriate, administer vaccinations for patients travelling abroad Promote and deliver evidence-based care for patients presenting with aural conditions Assist senior practitioners in providing minor-surgery sessions
