Summary
Assessment of new patients referred into the AMHT Brief follow-up work with patients who require that work following assessment When working with people who have been admitted to hospital, actively working alongside inpatient services to support good quality discharge planning Holding a caseload of patients who require longer-term follow-up from the AMHT Multi-professional and multi-agency liaison with other services in the local community Covering Duty on a roster with colleagues Being on a rota to cover some out-of-hours work as required Ensuring paperwork (including care plans and risk assessments) is up-to-date and of a high quality You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and Occupational Therapists and hold your development as central. We offer regular supervision and you will be encouraged and supported to undertake any training relevant to your professional development. In terms of hours we would prefer 30 to 37.5 hours per week with some weekend working- we welcome flexible working so please call us to discuss how we may be able to accommodate other working patterns. You will not be required to work night shifts.
