Summary


An exciting opportunity has arisen to join the proactive and enthusiastic Rapid Response Team (South) is looking to appoint a highly motivated Clinical Lead Nurse to join our Rapid Response Team. The post holder will work as a member of the multi disciplinary team supporting the Team Leader and her deputies in the delivery of the Rapid Response Service, supporting the development of the Rapid Response Team and raising the profile of the Rapid Response Service.

Rapid Response’s aim is to preventing hospital admission or facilitating early discharge from hospital by providing intensive packages of care to manage acutely and chronically ill patients at home.

Rapid Response is a multi-disciplinary team with support workers, physiotherapists, occupational therapists and Nurses providing integrated skills and sharing knowledge.

The post holder will work collaboratively with others and support members of the team in the delivery, and management of complex health issues.

In particular they will be responsible for ensuring the timely provision of a high standard of assessment, advice, planning, implementation, training support and evaluation of care for Rapid Response, 2hr Urgent Community Response and Virtual Wards.

A broad range of Nursing knowledge, interventions and skills are required. With a particular emphasis on the care of the respiratory patient at home.

You will have experience of working collaboratively within a community or acute setting. The post-holder will have excellent assessment, and intervention skills. You will be able to carry out comprehensive assessments and make autonomous decisions based on the data obtained. You will have the opportunity to provide client-centered assessments and interventions for a caseload of patients with acute illness.

You will also offer clinical supervision/support to other Nursing and support staff. The role requires good communication skills and an ability to work as an effective member of a multi-disciplinary nurse led team as well as with other external agencies.

A high level of inter-professional working exists in the Rapid Response Team giving the opportunity for you to develop and transfer your clinical skills.

This role will have a focus around the clinical governance of the service.

• Supporting the delivery of an integrated approach to patient care across all professions.
• Identifying and addressing areas of clinical development across the service.
• Providing training from their own area of expertise.
• Managing the performance of staff.
• Managing complex issues in the care of patients such as safeguarding risks or self neglect.
• Ensuring safe staffing levels are in place
• Supporting the wellbeing of staff.
• Deputising in the absence of the Team Leader
• Facilitating effective relationships with internal and external stakeholders such as other primary care teams and GP Practice staff.

This role would suit an individual with leadership experience and a dynamic mindset regarding how the service can best meet it's current and future objectives.
