Summary
Job Specification In order to alleviate pressure within our 6 member practices, West Leeds PCN undertakes to provide a domiciliary acute service each day so that our GPs dont have to do these visits. Our acute home visiting service is multi skilled, comprising Advanced Clinical Practitioners, Physician Associates and a paramedic; these staff members also work in areas other than acute so they have a mixed remit. We need an additional nurse / ANP member of staff to bolster overall capacity for this service; an experienced nurse will bring certain skills to the team that we don't currently have.
