Summary

To support service users with a range of needs under the supervision of a clinician. Under the direction/supervision of clinicians to assist with screening of referrals and client assessment. Work with service users to facilitate access to primary and secondary healthcare services. Assist members of the MDT to prepare deliver training and group sessions. Following assessment/review by clinicians assist to monitor services users holistic health and report any changes to the appropriate clinician. Undertake tasks delegated by supervisor and team manager. Support the multi-disciplinary team to deliver safe person-centred care. Develop resources for the team and services users; including accessible information. Create and maintain data to support effective running of the team. Ensure that the individuals rights and choices are actively promoted and supported to ensure they ae appropriately acknowledged. Provide support and assistance to professionals within the team. Liaise with health & social care agencies and care homes to meet services users needs. Visiting service users in a variety of settings including their home, work and day placements, hospital settings and group home, after carrying out an appropriate risk assessment. Maintain accurate records in accordance with policy and procedure. To have an understanding of the risk factors and health & safety issues involved in the provision of care to services users, families and self. To adhere to protocols, policies; including local Lone working policy. Assist to develop and maintain robust and up to date care plan and risk summaries. To develop a rapport with service users through professional support and collaboration within appropriate boundaries. To assist service users to maintain activities of daily living. To raise any concerns regarding service user or care provision promptly.
