Summary

Job Purpose/Summary To act within professional boundaries, working alongside a team of clinicians, social prescribers, health & wellbeing coaches, administrators and facilitators in general practice within the Nuneaton & Bedworth Primary Care Network (PCN). You will improve patients health outcomes and the efficiency of the Primary Care team by providing coaching to those who are not yet ready to fully benefit from social prescribing. Offering personalised support to patients/carers and families to support them to actively participate in their own healthcare. Through face-to-face contact or virtually by both group and individual circumstances. The postholder will be an integral part of the PCN team, working in allocated practices across the Primary Care Network, as well as being a valued member of the Personalised Care team. Key Responsibilities 1. Manage and prioritise a caseload, in accordance with the health and wellbeing needs of the population through taking an approach that is non-judgemental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues. Where medical treatment needs review or modifying, the Health & Wellbeing Coach will refer people back to other health professionals within the PCN or practice. 2. Utilise existing IT and MDT (Multi-Disciplinary Teams) channels to screen patients, with an aim to identify those that would benefit most from health coaching. 3. Provide personalised support to individuals, their families and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently. 4. Coaching and motivating patients through multiple sessions to identify their needs, set goals and supporting patients to achieve their personalised health and care plan objectives. 5. Providing interventions such as self-management education and peer support. 6. Supporting patients to establish and attain goals that are important to the patient. 7. Supporting personal choice and positive risk taking while ensuring that patients understand accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses. 8. Organising, running and facilitating small group sessions, including coordinating input from health care professionals. This currently includes regular Walk and Talk sessions in Nuneaton and Bedworth, but includes a requirement to identify future opportunities for new groups. 9. Working in partnership with the Social Prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing. 10. Increasing patient motivation to self-manage and adopt healthy behaviours. 11. Work with patients with lower activation scores to understand their level of knowledge, skills and confidence (their Activation level), when engaging with their health and wellbeing and subsequently supporting them in shared decision-making conversations. 12. Utilise health coaching skills to support people with lower levels of activation to develop the knowledge, skills and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. 13. Support clinicians in facilitating group consultations including virtually.
