Summary

The postholder will also work without direct supervision and deputise for the clinical team leader in their absence: Principle duties to include: Liaise with Education staff regarding the students Develop and maintain communication with people about difficult matters and/or in a difficult situation Have some knowledge about and be able to use alternative communication means used by the children e.g. makaton, PECs Responsible for communicating and receiving sensitive, complex, and sometimes contentious information relating to children and young people, relatives, and carers whilst ensuring confidentiality. Will use developed persuasive, motivational, negotiating skills, where agreement and cooperation is required, or were there may be barriers to understanding. Establish effective, appropriate communication processes with parents/guardians and carers. Communicate effectively with a wide range of health, education and social care professionals across professional boundaries. Responsible for communicating information both within the organisation and outside it. Establish and maintain effective channels of communication within own team and wider school team Participate in and contributes to meetings and working groups that inform service delivery, the organisation and partner agencies as appropriate. Take appropriate action if any child is identified as at risk. Act within local and national Safeguarding (Child Protection) Policies and Procedures. Responsible for ensuring the team leader and other members of the team are up to date with all matters affecting the service provided to the child and family. Accountable for the assessment and interpretation of information regarding child/young persons conditions and taking the appropriate action Ability to perform audit and research within the scope of the post. Contribute to the planning, development and implementation of systems and procedures that ensure that the Special School Nursing Service delivers responsive care within the allocated resources. Ensure that own role and responsibilities are clearly defined and the clinical competencies required for these are identified and met. Responsible for providing cover for the Clinical Team Leader as required. Support Clinical Team Lead in provision of expert and knowledgeable advice and support to colleagues within the service and other professionals Responsible, in conjunction with the Clinical Team Leader for the planning and organisation of the Special School Nursing services including allocating and re-allocating staff according to child/youngpersons need and the demands of the service. To escalate issues affecting day to day work within Special School nursing to Clinical Service Manager or appropriate other in absence of Clinical team lead Required to carry out physical procedures with the child/young person which may include catheterisation, change of tracheostomy breathing tubes, and replacement of Percutaneous Endoscopic Gastrostomy (PEG) feeding tubes etc (Following training and achievement of competency.) Required to have developed skills in the use of various equipment that requires accuracy within a narrow margin for error e.g. ventilators, saturation monitoring, enteral/nasogastric/nasojejunal feeding pumps etc. Driving to different school sites and attending meetings off site. Basic keyboard skills. To maintain a role in the delivery of clinical services as appropriate to the needs of the team Supports the development of quality initiatives i.e. Essence of Care, Every Child matters, National Service Frameworks for children Provides specialist clinical guidance to staff and other agencies Participates in the management of emergency situations including major incidents Participates in child protection case conferences, network meetings, and other multi-agency meetings and supports team members in the safeguarding process. Participate in the delivery of a holistic, autonomous & clinical nursing service. Make initial assessment, to plan & implement care, appropriate to the physical, psychological and social needs of the child & family. Empowering families to meet the childs needs at school and work in partnership with them. To have sound practice knowledge developed through experience within the specialist practice area and have the ability to advise clients on individual treatment programmes and their potential outcomes. Assess complex health and well being needs and develop, monitor and review care plans to meet those needs. Identify patient specific goals and interventions required to maximise health and wellbeing and develop effective approaches to meet the needs of child/young person/family. Support the development of quality initiatives e.g. Essence of Care. Evaluate the effectiveness of care and reviews care plans accordingly. Respect individual wishes, beliefs and consent to treatment ensuring dignity is maintained At all times. Determine and record treatment plans which are consistent with the outcomes of an assessment, consistent with the childs/young persons/families wishes and views in collaboration with the other agencies and professionals. To ensure the promotion of evidence based practice by reviewing and monitoring standards of care in line with local and national guidance and research. Required to liaise with other practitioners within childrens services locally and nationally to communicate good practice and ideas with the aim of each child/young person receiving the best possible care Act as an effective member of the Education, Therapy and Special School Nurse Teams by attending meetings and contributing to implementation of agreed standards of care. Responsible for ordering and maintaining levels of equipment needed for child/young persons care. Responsible for ensuring the safe use of equipment. Responsible for ensuring the annual servicing of equipment. Responsible for ensuring and monitoring cleaning of equipment. Responsible for ordering stock paracetamol and adrenaline from the Clinical Team Leader in line with set Standard Operating Procedures. Provide cover and support to other special school nurses as directed by the clinical team leader Participate in clinical supervision to sustain and develop practice, utilising the service to manage all aspects of personal, professional and work related issues including distressing and stressful situations. Plan a teaching programme with the head-teacher, to run throughout the school year. This will offer an opportunity for all school staff to gain the knowledge, skills, and understanding to carry out individual childrens nursing/personal care as required. Provide a teaching programme which involves specialists from different fields, including the school nurse herself, and other professionals e.g. therapists, paediatricians, infection control teams, in order to meet the training needs of the school staff. Be available to offer support, teaching and advice to school staff. Maintain a record of staff training relating to nursing care of the children, and ensure that an annual update is provided. Act as a mentor for student nurse learners (HE) and other college (FE) students who are placed in the school as part of their education. To maintain own knowledge and skills through continuing professional development and to share knowledge, expertise and act as a resource to colleagues and other health and social care workers. Responsible for ensuring that band 5 nurse and health care assistant receive Clinical Supervision. Provide professional advice and support to members of the team and other members of the multi professional team. Facilitate team cohesion through being a responsive, pro-active and flexible practitioner. Participate in the Trusts Performance Appraisal Scheme. Develop close working relationship with Practice Development Facilitator and peers across Surrey. Support learning by recognition of own and others learning needs, interests and styles. Gain feedback from learners and relevant others on the effectiveness of learning and development and ideas for how it might be improved. Reflect on and evaluate the effectiveness of learning. Discuss own evaluation with the team and agrees how learning and development might be improved in the future. Provide data about activity and caseload analysis as required. Ensure personal documentation and patient records are maintained effectively whilst being aware of the needs for the patients confidentiality and the Data Protection Act. Responsible for keeping accurate record of activity. Responsible for recording all personally generated information including maintaining accurate staff records, actions required etc Ensures that all staff have access to IT and facilitates its use wherever possible to record patient and clinical data, this includes supporting training in IT and specific systems Support the development of audit systems within the organisation to ensure continued evaluation of clinical practice by leading and facilitating clinical audit programmes and research in line with clinical governance priorities. Acts as an autonomous practitioner, accountable for own professional actions. Responsible for recognising the needs of the service, identifying risk and liaising with the clinical team leader to provide appropriate support for the Special School Nursing Service. Ensures that accurate and contemporaneous records are kept by self and staff in the team. Will deputise for the clinical team leader in their absence
