Summary
This is an important role for the Pathway and System Integration Business Unit, which focusses on tackling health inequalities and in particular leads on the programme delivery of the CORE20plus5 standards across our work plans, as well as coordinating several system wide groups such as the Healthy Adults Population Board and associated subgroups. The post holder will provide expertise and leadership to the system both in regards of specific contracting and service provision in these areas, and in terms of strategy and direction, advising and reporting to senior leadership on direction of travel and progress and coordinating efforts to tackle health inequalities across multiple population segments. The post holder will be confident in data analytics and will be capable of identifying and driving improvement based on both qualitative and quantitative data. The post will be a key liaison point with the WY health inequalities programme and population health management team in this respect. The post holder will have the skills and behaviours to develop strong relationships across a wide range of partners including the VCSE and Leeds City Council. They will have the resilience and tenacity to navigate these relationships in the current financial context, preserving and promoting partnership working whilst delivering financial sustainability and best value. The post holder will be passionate about prevention, improving population health and particularly about improving the health of the poorest in Leeds the fastest. They will deliver their work portfolio with a dedicated focus on this direction of travel but also be open and flexible if required to work with other population groups or service areas in line with emerging priorities.
