Summary
Applicants must be 1st Level Registered Nurse with current NMC Registration. Role requires a registered nurse with specialist skills to assess patient needs, analyse and develop a plan of care within a multidisciplinary framework. To provide realistic and occasionally intensive support to patients within a community setting. The role requires excellent communication skills and the ability to develop and build relationships with partner agencies. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
