An exciting opportunity has arisen in Mid- Essex for an experienced band 6 Occupational therapist to join our vibrant multi-professional community Urgent Community Response Team (UCRT) Admission Avoidance Team. Every member of our team is focused and passionate about ensuring that all our patients receive the highest standards of care, which is delivered with dignity, respect and compassion.

Flexible working hours on a full-time basis are offered (Long days will be considered) to ensure the service is covered. A valid, clean driving license and use of a vehicle is essential.

The role of the UCRT therapist is to carry out rapid response functional assessments and interventions to support patients to remain safe and independent in their own home and support recovery. The post holders will need to have a high degree of professional autonomy and be able to make critical/clinical judgements using expert knowledge, skills and experience in order to provide a gold standard of holistic care including complex management of patients with multiple health conditions as the Band 6, you will be responsible for working autonomously during shift, feeding back to Clinical Leads within UCRT. You will be expected to contribute to the service as a whole with MDTs, service development, supporting the trust and attend service meetings.

The post requires you to be able to carry equipment and travel though out the whole area commissioned.

UCRT is a community admission avoidance service available 7 days a week that supports patients in crisis to stay at home rather than being transferred to an acute setting. The UCRT team provides specialist Therapy interventions and nurse led care which has continued to demonstrate positive outcomes for patients and their families with regards to front facing, holistic, patient-centered care.

Regular professional supervision and CPD will be provided. The post holder will be well supported by the UCRT Therapy Lead, Operational Lead and the wider nursing team, This post offers excellent opportunities for the candidate with regards to on-going professional and self-development, training (and access to further education.) Within the service we have Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistants and we are supported by a Frailty consultant.
