Summary
To work with clinical staff within the acute hospitals to support and ensure the provision of safe, high quality services for individuals with learning disabilities using inpatient services, minimising risk, incident and error and fostering a culture of learning and reflection. To support implementation of the Directed Enhanced Service (DES) Annual Health checks for people with Learning Disabilities, through provision of specialist education and support to Primary Care Staff to ensure that practices are able to meet its requirements and to promote uptake of the DES amongst practices across West Sussex.
