See attached Job Description and Person Specification for a full description: Be present at the point of access to Health and Social Care, contributing to the multidisciplinary screening of incoming referrals at the Common Access Point and carrying a caseload of rapid response and urgent enquiries. The team are also an integral part of the Home First MDT screening; providing same day response and reablement to people discharged from hospital.o Hold varied caseload and work as part of a multi-disciplinary team to support people to maximize independence. You will carry out functional, holistic, and specialised assessments, and interventionso Interventions may include making recommendations for minor and major adaptations to clients homes.o Supervise junior staff (Health/Local Authority) and support student placements.o Participate in audits and contribute to service development, maintain professional standards and governance.
