Summary

Patient Care Reconcile patients medications on admission to the hospice, ensuring an accurate, current list of medications is available and documented in a timely manner. Escalating any issues found during the medicines reconciliation process to the relevant health care professional. Document clearly in the patients electronic records and medication charts To assess the suitability of the use of patients own medications, ensuring patients consent to use or dispose of their medication whilst in the hospice and to arrange the efficient and prompt supply of medicines where needed. To assess whether the patient has any difficulties managing their medicines or concerns about their medicines and work with the patient, family and multidisciplinary team to address these To support discharge through working as part of the ward based team, including ordering of urgent items, ensuring timely ordering and delivery of TTOs (To Take Out) and assessing patients own drugs to use on discharge Contribute to the efficient discharge of patients by ensuring the patient has the medicines they need when they are discharged, and understands how to take them and get continued supplies Stock Maintenance Support the ward in managing and maintaining safe and efficient stock levels of pharmaceuticals To undertake ward top-up services, in accordance with Service Level Agreements (SLA) Work collaboratively with our SLA provider to ensure processes are followed and any issues identified Dispose of and collect ongoing data on returned and wasted medicines Maintain stock levels and tidiness of medicine cupboards, ensuring waste is minimised To ensure appropriate maintenance of the bedside medicine cabinets by actively putting away patient specific dispensed medicines and PODs (patients own drugs), ensuring they are transferred with patients in the event of bed moves, being aware of when medicines will run out and ensuring continuity of supply. To ensure any stopped or suspended medicines are removed from the bedside medicine cabinets in a timely manner. Cleaning of bedside medicine cabinets when vacated or when spillages etc are found Miscellaneous Assist the authorised witness in the destruction of controlled drugs no longer suitable for use. Assist with the medicines audit programme Help co-ordinate medicines management updates and training Administrative tasks e.g. end of month paperwork filing, photocopying pharmacy documentation, scanning and filing pharmacy documentation to ensure records are fully maintained To participate in other initiatives designed to improve the efficiency of medicines management activities as necessary and in line with competencies To identify good practice in medicines management which could then be used to improve services to patients Education and Training Assisting the Pharmacy Team to deliver Medicines Management training Be responsible for own continuing professional development in line with professional requirements in order to continue to update knowledge and skills. Maintain own mandatory training requirements Risk Management and Clinical Governance Ensure all medication incidents are reported in accordance with Hospice policy Assist where appropriate in investigating errors in prescribing, dispensing or drug administration Assist the team in ensuring compliance with the CQC standards for medicines management Information Technology Be competent in using basic computer programmes Access the internet on a regular basis to ensure access to up-to-date information on medicines related issues and access work emails To be competent in using relevant parts of electronic care record systems in order to effectively carry out the job. Finance/Resources To ensure stock levels are maintained, stock is rotated and waste is kept to a minimum To highlight and make suggestions to ensure best use of Hospice finances in relation to the cost effective use of medicines.
